1. Go to “mobile.entryapp.ca”
2. Login or register
3. Tap the “Questionnaire” button
4. Agree to the legal notifications presented by the facility (if any)
5. Enter the resident you’re visiting (if applicable)
6. Fill in the questionnaire
7. Use the QR code to enter the facility
